- Download and fill out our Credit Application and Regulatory Information Sheet (Required for Controlled Substances).
- Fax your Credit Application, Regulatory Information Sheet, DEA Certificate and State License to 1+ 614-297-8224.
- Your information will be reviewed and processed within 48 hours.
- Upon approval, your account will be established.
- Your personal account manager will contact you to answer any questions regarding your account.
